Chronical Disease Management

Chronic disease is inarguably the biggest problem faced by healthcare systems all over the world. These diseases usually develop slowly, last long periods of time, and often are never cured. In most cases, there is no cure. The long-term effects may be difficult to predict. While some conditions cause few problems, others cause only episodic problems or symptoms that can be controlled with medication but need regular monitoring. In some cases, a chronic disease may severely limit a person's ability to work, go to school or take care of routine needs. Examples of chronic diseases include, but are in no way limited to: diabetes, congestive heart failure, asthma, hypertension, chronic kidney disease, depression, irritable bowel syndrome, arthritis, fibromyalgia, Parkinson's syndrome, and multiple sclerosis - just to mention a few.

Though chronic diseases are lifelong, proactive disease management and healthy lifestyle choices can help one stay ahead of the curve. Although a lot of information is available online, it only seems like medical jargon, which is difficult to comprehend and implement. Understanding the needs concerns and difficulties faced, we at ATHS, are providing this Chronic Disease Management Program (DMP) to aid and enable you to manage these conditions.

Our Chronic Disease Management Program covers the following areas:

1.continuous assessment and monitoring of your chronic condition

2.knowing how to recognize and respond to complications associated with chronic disease

3.understanding and using medicines and treatments effectively

4.coping with morbidity and irreversible conditions associated with the disease

5.coping with frustration, fatigue, pain, sleep and sexual health related problems

6.diet counselling

7.making decisions about when to seek medical help

8.working with your doctor(s) and other care providers

9.managing work, family and social activities

10.fitness counselling by suggesting appropriate exercise

11.communicating effectively with family, friends, and health professionals

12.understanding and adjusting to new treatments.

Note: The Self-Management Program will not conflict with existing programs or treatment. It is designed to enhance regular treatment and disease-specific education such as Better Breathe easy, cardiac rehabilitation, or diabetes and hypertension management instruction. In addition, many people have more than one chronic condition. The program is especially helpful for these people, as it gives them the skills to coordinate all the things needed to manage their health, as well as to help them keep active in their lives.
